Betrayal and Loss: Lincoln returns home to his surrogate Black Mob family, led by Sammy Robinson. Sammy is in debt to the Italian Mafia boss, Sal Marcano. Lincoln reluctantly agrees to help with a heist at the Federal Reserve to settle the debt. However, after the successful heist, Marcano betrays and brutally murders Sammy, Lincoln's adoptive brother Ellis, and their associates, leaving Lincoln for dead.

Cassandra: The leader of the Haitian gang, seeking to reclaim her territory taken by the Dixie Mafia under Marcano's control.
Vito Scaletta: The protagonist of Mafia II, now down on his luck and seeking to rebuild his standing after being exiled to New Bordeaux.
Thomas Burke: An Irish mob boss whose son was murdered by Marcano's men.
Taking Over the City: Lincoln systematically targets Marcano's rackets and territories, weakening his grip on New Bordeaux. He assigns these territories to his underbosses (Cassandra, Vito, and Burke), forming a new criminal organization. The player must balance the distribution of territories to keep the underbosses loyal, as neglecting them can lead to consequences.
